How to Join IDFY
- WHO may apply?
- Middle and High School Students
- WHAT must I do?
- Submit to an initial voluntary drug screen (FREE of charge!).
See below for more details.
- Bring the following "items" to the initial drug screen with you:
- A parent or legal guardian
- Don't want the folks tagging along? Then bring a Picture ID and a signed
parental consent form. This form can be obtained from your school counselor, by contacting us (and one will be sent to
you) or just click here to print one out NOW!
- WHERE do I go?
- Initial voluntary drug screens are held at the following location:
Pathology Laboratory
830 Bayou Pines.
Lake Charles, Louisiana
- WHEN are they held?
- Monday through Friday between 1:30PM and 4:30PM. You must call (337) 436-9557 to
make an appointment prior to showing up.
